2011/2012 Registration Instructions

 

Below is a two-step process outlining the procedures for registering for USA Hockey and then the Yellowjackets programs of your choice.

Step 1 – USA Hockey Registration

To Register

1.  In order to register for the Fall Season you will need to register with USA Hockey for the 2011-2012 Season if you haven't already done so.

2.  Go to www.usahockeyregistration.com.

3.  Click the red box on the right that says 'Click Here to become a USA Hockey Memebr'

4.    Select 'Ice Players and Coaches' check the box to confirm you are 18 years of age or older and click 'continue' for the link to take you through the registration process for 2011-2012.

5.    Please read the waiver and check 'I accept box and enter your initials.'

6.    Be sure you review the information and either 'edit' or continue'. Confirm the state is 'IL' and click continue.

7.    Enter your credit card information. The total charge is $43 ($30 to USA Hockey & $13 to AHAI).

8.   At the end of the process you will be given a USA Hockey confirmation number consisting of 9 digets and 5 letters. The number is very important and is REQUIRED to register for the fall season. Print the confirmation page and keep a copy for your records.

 


Step 2 – Fire Wagon Yellowjackets Hockey Registration

1.    Log on to the Yellowjackets Web site www.firewagonhockey.com.

2.    Log into the site by using the login link (you will need your password that you used to register previously - or if you cannot find it, request a new password and it will be e-mailed to you).

3.    Click on the Registration Information 2011/2012 link and review the information and options.

4.    Once you have determined the programs you wish to enroll, click on the Registration link.

5.    Choose either Youth or Girls depending on the player you are registering.

6.    Fill in all information and choose your enrollment options.

7.    Remember to enter the 9 number 5 alpha confirmation into the “USAH Confirmation #” field.

8.    Click the link to save.

9.    If after pressing 'SAVE', an error is found on your registration form, you will be re-directed to the top of the registration form.  Please scroll down and fix errors found on registration form and click on 'SAVE' once more.

10.  After pressing save, you will be re-directed to a registration successfully completed page and will receive an emailed registration form in 2 - 5 minutes. If you do not receive an e-mailed registration form within 5 minutes, please check your Spam/Junk mail folder.

11.  Steps 4-10 can be repeated for additional players you wish to register.

12.  Print, sign and mail your registration documents, including your USA Hockey confirmation page along with your Deposit payment of $275 to Fire Wagon Hockey Inc. PO Box 1854, Crystal Lake, IL 60039. Please put Child's/Children's name on your payment.

 

It is extremely important to include a copy of your USA Hockey confirmation page, which includes a bar code, in your mailed documents. To automate the registration process the Yellowjackets registrar requires this bar code for scanning.

 

Registration Packets should include:

  • Signed copy of registration form.
  • USA Hockey Registration confirmation page with bar code.
  • Deposit payment of $275
  • USA Hockey Youth Transfer form. (If necessary). This form is to be used for non-US citizens registering to play with the Yellowjackets in 2011/2012

 

Payment must be enclosed for your registration to be considered complete!
